Lake County residents who qualify for Phase 1B and are interested in receiving the COVID-19 vaccine are now able to provide name and contact information to be placed on a vaccination clinic scheduling list.
Those eligible include those who are (1) 65 years of age and older, and/or (2) have severe congenital developmental or early-onset medical disorders. Those who complete the form will then be contacted at a later date with instructions to schedule an appointment for COVID-19 vaccination at the appropriate time. Persons working in K-12 schools returning to in-person learning on March 1st will be vaccinated in coordination with their school districts and should not complete this form.
To complete the form, residents should go to www.lcghd.org and click the button for COVID-19 Vaccine Information. The form is linked under the Phase 1B heading.
COVID-19 vaccine is extremely limited, and LCGHD does not know how many doses will be received in the coming weeks. For this reason, completing a form does not guarantee a COVID-19 vaccine appointment.
If you are in Phase 1B, and have previously contacted LCGHD or submitted an inquiry, you will need to submit information in this new form to be placed on the list. Please complete one submission per eligible person.
Information is evolving, and more information will likely be provided next week.
General questions regarding COVID-19 can be addressed to Lake County General Health District at covid19@lcghd.org or (440)350-2188. Visit www.lcghd.org for the most updated COVID-19 information.
