The Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) are community documents used to assess Lake County health needs and create strategies to improve the health of Lake County residents.
The 2025 CHNA is now underway! Make your voice heard and take the community resident survey in English (https://bit.ly/2025LGCHNA_English) or Spanish (https://bit.ly/2025LGCHNA_Espanol) between Monday, December 19, 2024 and Tuesday, December 31, 2024.
These documents are developed in partnership with a wide variety of organizations representing many different sectors of the community. Partners meet quarterly to review new health data, update progress on health improvement strategies, and revise the health improvement plan when needed. See the full list of partners.
Community Health Needs Assessment (CHNA)
Local health departments that are accredited from the Public Health Accreditation Board (PHAB) are required to conduct a community health assessment every five years, and non-profit hospital systems must complete an assessment every three years to retain their Internal Revenue Services 501(c)(3) status. In 2019, University Hospitals (previously Lake Health), and LCGHD completed their first joint community health need together.
Identifying various factors that influence health at a population level is a large focus for both the public health and health care systems. The community health needs assessment is a great tool to figure out how these factors impact the health-related needs of a community. Health needs are identified through a collection and study of a wide variety of information and data.
During the three-year CHNA cycle, additional data will continue to be collected and reported to provide a great understanding of current health needs.

Community Health Improvement Plan (CHIP)
After a CHNA is completed, the partners review the results, prioritize health needs, and identify strategies to impact health needs in Lake County. Partner organizations take the lead on strategies most aligned to their organization’s work and create measurable objectives and action steps to monitor progress.
Strategies may be modified in response to organizational changes, funding priorities, or emerging health needs. A quarterly progress report is developed and published.
