Lake County Connections, a senior outreach program of the Lake County General Health District, and the Unite Ohio network, a coordinated care network, have teamed up to use technology to connect and support the health and well-being of senior communities in Lake County. They aim to address the unmet needs of the senior population in the county through electronic referrals.

The program provides a central point of contact where health and social service providers can securely access and refer people to needed services while monitoring progress and measuring outcomes. The partnership includes collaboration with other nonprofit organizations within Lake County to bridge the gap between health and social services and support community members’ health and overall wellness.

A virtual library of updated peer-specific information has been created with the intent to enhance already successful Health District programs. Connections has created a portal of active links to assist seniors in navigating some of our county’s health services, senior centers, transportation providers, and social service networks.
