Lake County General Health District (LCGHD) is now scheduling dedicated COVID-19 vaccine clinics for 5 to 11 year-olds at its building located at 5966 Heisley Road in Mentor, Ohio. Due to the limited supply of vaccine received, vaccines will be given by appointment only. LCGHD will only be vaccinating 5 to 11 year-olds at these clinics. No walk-ins will be accepted. Parents may call to schedule an appointment at 440-350-2188, or schedule an appointment on ArmorVax (https://armorvax-app.azurewebsites.net/Account/Login). A parent or guardian must be present with the child and sign a consent form. Parents are also asked to bring their photo identification and insurance cards if able.

- November 13th from 1:00 p.m. to 3:00 p.m.
- November 16th from 2:00 p.m. to 6:00 p.m.
- November 18th from 1:30 p.m. to 4:00 p.m.
- November 19th from 9:00 a.m. to 4:00 p.m.
- November 23rd from 9:00 a.m. to 6:00 p.m.
- November 30th from 9:00 a.m. to 12:00 p.m.
