The Lake County General Health District (LCGHD), with University Hospitals, is currently conducting the 2025 Lake County Community Health Needs Assessment (CHNA) and want to hear from you, the public. This assessment is a way for LCGHD and University Hospitals to hear from the public about their health-related concerns like access to healthy food, smoking cigarettes, substance use, mental health challenges and other areas. Public engagement is key in this process, which is why folks from across Lake County are invited to complete a community resident survey to let their voices be heard.
The community resident survey is now available for Lake County residents 18 years of age and older and will cover a range of health-related topics. The survey is available in both English and Spanish and can be accessed by the following direct links https://bit.ly/2025LGCHNA_English (English) and https://bit.ly/2025LGCHNA_Espanol (Spanish). The survey takes less than 15 minutes to complete and is both voluntary and anonymous. The survey will be available until at least Tuesday, December 31, 2024, and can be taken on your computer, tablet, or smart phone.
The purpose of the assessment is to understand the current health needs of Lake County residents, as well as understand both where and how the health of the community can be improved. The assessment includes a review of national and state data sources, the community resident survey, and targeted community focus groups, which are currently happening across Lake County.
“The population’s health is divided by the social determinants of health resulting in vastly different qualities of life. The resource-rich and well-connected are not only far more likely to receive care when they need it, but they also start off with advantages that have a tremendous effect on health — in housing, employment, stress levels, food security, social capital and more,” said Health Commissioner, Ron H. Graham. “Health starts in our homes, schools, workplaces, neighborhoods, and communities. We must partner to create social and physical environments that promote good health for all.”
Hard copies of the English and Spanish language surveys will be available at Lake County General Health District, 5966 Heisley Road, in Mentor, by request, and at select local partner buildings. If your business or organization would like to assist LCGHD with distributing the CHNA survey, contact the health district at 440.350.2543 to pick up hard copies.
